1) Yes. Reaction attack is a part of the melee sequence
2) I guess it does. Unlike Overwatch, it doesn't specify that the opponent's activation ends if the attack is successful
3) Looks like it, but the charged model can make reaction attacks as well. It also doesn't remove the ready marker so...

I confess I do love me some saturation. If manufacturers get away making models with cartoon-ish proportions, why can't I go along and sneak in cartoon-ish colors as well?:sneaky:
Your fighters can be equipped that way, but can only use of the weapons to shoot during an activation. Also, just in case, I don't think there's any way to have a storm bolter at the very start of the Campaign.
Looks like they added to a sentence to the Melta trait: "If a weapon does not have a Short range, then the Melta trait affects all attacks made with this weapon". I guess this should apply to melta bombs/traps.
