Hive Scum don't stick around for the long haul, so aren't a great early investment for most gangs, I expect.

And there are a fair number of Hangers-On that are economically better choices than a Rogue Doc, but then insurance is insurance, and many times it won't pay out, but when it DOES pay off.... well..

That is the main draw of different ammo types. A useful situational tool but better ammo rolls to reload all the time.

For the record, ask my arbitrator about this. He agrees with you that you can use the smokes to reload the krak. Says it's a known thing for a while now...

Depends.
Last few campaigns I played whenever I had a Rouge doc that gang never had a critical injury so their ability never came up.
Maybe the gangers just refused to get that sick to avoid the surgical knives.
