Typo fixes, fixed some issues in the Serious Injuries table, swapped out a bit of art

The table of contents wasn't being generated properly (thanks, LibreOffice...). Switching it from Outline to manually picking up H1, H2, and H3 seems to have solved the problem. Otherwise the contents of the document are the same.
